Focus Care Hospice provides care and support to terminally ill patients and their families. Patients spend their remaining time in the comfort and dignity of home. Specially trained hospice staff provide nursing care, pain and symptom management, spiritual support and bereavement counseling as well as support groups for patients and families. Research shows that most people would prefer to spend their final days at home, free of pain and surrounded by the people they love. Today it is possible to get the needed support at home through hospice care. FocusCare Hospice focuses on promoting comfort and self-determination, so that patients can participate in making decisions about care at the end of their lives. Families are closely involved in decision-making, and hospice provides counseling, education, short-term respite care, and bereavement support for those close to the patient. Hospice services can be provided in the home, in a nursing home, in an assisted living facility or occasionally in the hospital. |

| Home health care is provided to homebound patients on a physician referral basis. To be eligible for referral a patient must be homebound and require skilled nursing. | Focus Care works with the medical community to care for patients.
